The Community Services Provider is responsible for providing direct care through implementation of Habilitation Plans. Specific duties and responsibilities include but are not limited to: Provide supervision and all direct care required to maintain safety and comfort of the person receiving services, Transport or accompany people receiving services on trips to community outings, appointments, or recreation activities for the purpose of community integration and exploration, Development and planning of inclusive community activities. High School graduate/GED with some knowledge of or experience working with developmentally disabled preferred. Possess a valid driver’s license.
