Conduct home visits with individuals who are legally blind to complete intake assessments and determine vision rehabilitation needs for the Adaptive Living Program cases. Manage Adaptive Living Program cases including communication with appropriate AVRE staff to initiate direct service provision of Vision Rehabilitation Therapy and Orientation & Mobility Instruction based on intake assessment. Provide Social Casework services to consumers including adjustment to blindness and access to community resources for a variety of social issues. Requires a bachelor’s in social work, or a bachelor’s or master’s degree in a related social/human services field with a minimum of one year of work social or related experience. $21.00 – $24.00/hr.
